Patient forms - Import and view the form in the patient record

When a form is completed by the patient, a notification appears at the top of the patient record. Only the requester and the people with whom the requester shares a mandate will be able to see the notification of the form completed by the patient and import it into the record. 

During import, if a clinical note is open on the screen, the patient form will be added to it. If no clinical note is open, a new clinical note is created automatically. The patient form will then be available within the note, like any other clinical tool.

If the Requester field is empty, all clinic users will be able to view and import the notification.

Steps to follow

1. In the notification at the top of the patient record, click on the Add to record button. 

 

2. When import of the patient form is complete, a notification appears in the bottom right-hand corner of the screen:

 

 

3. If a clinical note is open on the screen, the patient form will be added to it. 

If no clinical note is open, a new clinical note is created.

To view the contents of the form, click on the arrow next to the form title to open it.


  • Only completed and downloaded patient forms are displayed in the patient record.  
  • Patient forms have the same functionality as the other clinical tools. See Clinical tools overview for all the details.

If the form contains a calculation, the note must be saved so that the calculation can be updated and integrated, for example, in the Results summary box.

In the pilot phase, forms entered by patients are not integrated in the same section as clinical tools of the same name (e.g. PHQ-9, GAD-7) in the Clinical Tools summary box.
